Refresh Code of Conduct from v1.4 to v2.0 (#673)
Update Code of Conduct from v1.4 to v2.0 (see https://www.contributor-covenant.org/version/2/0/code_of_conduct/), and add a Contributor Covenant badge to the main README.md.
