Add notes from the 2019-06-06 GraphQL Working Group meeting. These notes include a new Action Items top section to easily refer to during the next meeting (related to #192).

Thanks to @BrunoScheufler for pairing with me on the note-taking!

I realized earlier that I had not yet signed the GraphQL Specification Membership Agreement and Code of Conduct. I've now signed and submitted that document through DocuSign.

@dblandin @BrunoScheufler Thanks for excellent notes and especially for action points.
I went ahead and merged this PR since I want to be able to post links to individual notes sections. We can always apply any changes, if needed, in separate PRs.
