Fix redirect to patch edit page on creation

After this refactor of the patch URL, when you create a patch, you're still redirected to the old patch URL, which no longer exists.

Thanks that looks like a good improvement. Just to be clear, it did work expected before right? Just with another (unnecessary) redirect happening?

Not for me - I got a 404 locally when adding a patch from the commitfest page prior to this change.
