We removed the StopContext method on schema.Provider, but it isn't documented in the upgrade guide. Providers should use the context-aware Configure property for schema.Provider instead.

Add some missing info to the SDKv2 upgrade guide.

We missed some sections and have some new sections that needed to be
added to the upgrade guide to v2 of the SDK.
